Photo Release

I hereby irrevocably consent to and authorize the unrestricted use and reproduction by Element Church, Inc. ("the Church"), or anyone authorized by the Church, of any and all photographs and/or video images which you have taken on this date(s) of myself and/or my child(ren), for use within publications, print ads, electronic and social media, or any other form. I release the Church from any liability for any violation of any personal right I may have in connection with such use. This consent is absolute and without limitation as to (1) the time within which any such photographs or video images must be used, (2) the nature and extent of their use, or (3) by whom they may be used. All services are live streamed and will need consent. If you do not consent to the use of your child's images, we will contact you about doing a private ceremony.
